We provide you with a detailed patient encounter form that you may use to submit to your insurance company to be reimbursed. Depending on your plan, the reimbursement is typically between 50-100% of the cost of the visit.

New York City (Manhattan, Brooklyn, Queens, Staten Island)

Nassau, and Suffolk county (including the Hamptons)

Westchester, and Rockland County

Bergen and Essex County New Jersey

Dr Edward A Kulich is committed to providing the highest quality pediatric urgent care for your children in the comfort and convenience of your own home or hotel room.

We do not contract with insurance companies and payment is expected at time of service.

We are not held responsible for any costs incurred by third parties such as labs or outpatient radiology centers or pharmacies or specialists. We do not charge for rapid tests done on site, but occasionally, more complicated tests are required to be sent to an outside laboratory, this is usually reimbursed by your insurance company and we are not responsible for the third party lab costs. We do not charge lab collection or lab handling fees.

Please note, that if your child is covered by Medicaid, we will not be able to provide our services since we are not Medicaid providers and it is against federal law to accept payment from patients that have Medicaid coverage. If your child has Medicaid coverage, please call you child’s health care provider or go to the nearest emergency room.
